Annual Wellness Visit / Health Risk Assessment

A wellness visit, from Care Connectors, gives members the flexibility and options to have their annual medical assessment conducted by our highly trained nurse practitioners in the comfort of their homes, in a centralized local Care Connectors clinic, or a primary care physician’s office. A wellness visit may occur at any time during the year and can be scheduled for Medicare Advantage (MA), Commercial Medicaid members who have obstacles adhering to scheduled primary care appointments.

Care Connectors’ proprietary process involves a comprehensive assessment that focuses on member engagement and clinical profiles, to gather the most accurate and updated data. Our new member telephonic Health Risk Assessment (HRA) survey, is designed to assist members find the right care program, thus allowing Primary Care Providers to provide personalized care.

In our telephonic HRA setting, Care Connectors can assess and provide a deeper insight into the patient’s medical history though our 1 on 1 interaction; allowing patients to share their current health conditions and allowing the provider to assess their needs. This allows our care coordination team to work alongside them and get the medical, behavioral, social care and supplies they may need.

The Care Connectors wellness visit and HRA are effective tools for facilitating timely gap closures and improving your care management initiatives through our integrated quality measures, including Star measures, HEDIS and CAHPS/HOS surveys. Through our user-friendly portal, you will have all the data you need at your finger-tips, and in real time.
