Full Time, Part Time We invite you to join our team! Care Connectors Medical Group is looking for a Full-Time or Part-time Board-Certified Nurse Practitioner to conduct an Annual Wellness Exam In-Home in the Denver and Colorado Springs, Colorado area.
Position overview:
Conduct comprehensive In-Home Annual Wellness Exams, in the Denver and Colorado Springs area, on Medicare population with chronic disease conditions, as well as assist in identifying and closing quality care gaps through labs and screenings. Educate patients on chronic health conditions, preventative screenings/tests, and lifestyle concerns. Make recommendations and communicate findings to Primary Care Physicians through documentation of outcomes.
Applicants must be local in Denver and Colorado Springs, CO
- Master’s Degree in Nursing from an accredited school of Nursing
- Board Certification with current unrestricted Nurse Practitioner (NP) License
- National Certification
- Full-time, Part-time, Contract
- Willingness to travel
- Minimum 2 years’ experience
- Previous Annual Wellness visit or In-home assessments experience preferred
- Bilingual in English and Spanish preferred
- Previous EMR skills preferred
Compensation:
- The position is an Independent Contractor and based on per visit.
- Mileage reimbursed
Pay
- Up to $140,000.00 per year
