Care Connector's Behavioral Health (BH) Network utilizes a population health approach to identify members. Our processes continually utilize screening tools, prescription data and an understanding of social risks associated with non-clinical factors. We utilize a variation of a collaborative care model that involves the integration of BH care managers and consultant psychiatrists, with the oversight from a prescribing NP for proactive management.
Our Approach
The BH care managers directly interact with members virtually or during an in-home assessment to set goals, provide counseling, monitor medications, and assess symptoms on a predetermined basis. The care team then reviews the member’s treatment plan and status with the psychiatrist consultant. Together the primary care team builds treatment strategies that may include referral to behavioral health specialty care supported by Care Connectors proprietary Care-DNA platform.
