Companionship (CarePal)
Care Connectors CarePal is a platform designed for support of older adults and provides a level of companionship/support, as well as needed assistance, identified through Annual Wellness visits and Care Management needs. CarePal pairs members with a ‘Pal’ that acts as a care guide and checks-in with members regularly to support member's growing need to connect with community as well as taking necessary healthy steps to maintaining an active lifestyle.
Care Connectors’ team will work alongside health plans and medical groups to address a member’s needs by identifying the right social risk(s) associated with their members. Not only do we utilize demographic and clinical factors but also by nonclinical factors, such as health-related behaviors, psychosocial factors, and patients’ social environment. We take into consideration the interplay between different health problems, actions of patients and clinicians, and social attributes in maintaining and enhancing a member’s health. This is enabled through our Care-DNA proprietary platform.
